By purchasing our products and services, you agree to be bound by the following Terms and Conditions. Please read them carefully.

1. IntroductionThese Terms and Conditions ("Terms") govern your use of the Alliance services and products, including the Alliance Consultation Package. By making a purchase, you agree to these Terms.  

2. Definitions

•       Company: Alliance MediRehab Associates, LLC

•       Prescription pharmaceuticals will be provided by the pharmacy of your choice.

•       User/You: Any individual who purchases or uses our services.  

3. Eligibility To be eligible to purchase our services, you must be at least 18 years old and capable of entering into a legally binding agreement.  

4.Our Services are NOT of the traditional medical model. Using an aligned medical model based on natural, wholistic, lifestyle modifications and natural care, our services emphasize educational and informational purposes as well as pharmacologic interventions to empower you in preventive and foundational self-care. Our virtual consultations and treatments are not a substitute for a physical examination, diagnosis, or treatment by your primary medical care provider but is offered in conjunction with your routine medical care. You should always seek the advice of a licensed healthcare professional regarding any medical condition you may have.  

5. Service Description

•       Alliance offers 1-on-1 Virtual Consultations or Evaluations: A personalized consultation with one of our health experts to assess your health status and develop a customized wellness plan.

•       Alliance Wellness Assessment: An in-depth questionnaire designed to identify key areas for improvement in your health.

6. Payment Terms

•       All prices are listed in US dollars and are subject to change without notice.

•     Payment is due at the time of purchase.

•     We accept all major debit and credit cards, digital wallets including Amazon Pay, Apple Pay, Google Pay, Cash App Pay, credit transfers and wires.  

•    You may use your Flex-spending or Healthcare Savings Account funds for our services.

The Reason Why We Do Not Accept Assignments with Insurance:

•    A question that many prospective patients have asked us is why we do not bill insurance companies directly, especially as most other health care providers do. We understand this situation can create a serious financial burden for some patients, and we're saddened we cannot bill your insurance like your dentists, car mechanics, day care centers, or anyone else. But it is a matter of fact. Here's why:

When clinics bill directly to health insurance companies, the doctors need to become participating providers. This means they have to be bound by which services the insurance company will approve and which services they

 won't, and how much they can bill for those services. Generally, insurance companies do not focus on billings for any preventive or wellness services. They are focused more heavily on the conventional model of healthcare, which is too often band-aid fixes and not the ultimate or optimal health model. Our commitment is toward a more foundational model, which works to address the root causes of your symptoms through specific nutrition and lifestyle recommendations.

• In the current healthcare environment, a doctor’s cost to provide services has increased, but payments of reasonable fees covered by the insurance is declining or may not be authorized. Meanwhile, profits and salaries for health insurance companies and their top executives continue to reach record highs. Unfortunately, we have found that we cannot be participating providers in the insurance networks and provide the time-intensive, quality, expert foundational care that we strive to provide. 

7. Refund Policy

•       Due to the nature of our services, all sales are final and non-refundable.

•       The consultation fee is NON-REFUNDABLE, as we dedicate substantial time and expertise to your assessment.

8. Confidentiality and Privacy

•       We are committed to protecting your privacy and maintaining the confidentiality of your personal information.

9. Intellectual Property

•       All content provided by Alliance, including but not limited to text, graphics, logos, and software, is the property of the Company and is protected by applicable intellectual property laws.

•       You may not reproduce, distribute, or otherwise use any content without our express written permission.  

10. Disclaimer of Warranties

•       Our services are provided "as is" and "as available" without any warranties, express or implied.

•       We do not guarantee that our services will be error-free or that you will achieve any specific results.   Results are most often reflective of your commitment and desire to embrace recommended strategies. We are here to support you in your efforts.

11. Limitation of Liability

•       To the maximum extent permitted by law, Alliance shall not be liable for any indirect, incidental, special, or consequential damages arising out of or in connection with the use of our services.

•       Our total liability for any claims under these Terms shall not exceed the amount paid by you to the Company.  

12. AI Use Disclaimer. Service Provider may use artificial intellignce ("AI") Tools as part of the creative or editing process, including but not liited to image culling, copy editing design enhancement or content generation. Service provider affirms that the final deliverables provided to Client are human-directed, reviewed and finalized, and remain the intellectual property of Service Provider unless otherwise assigned under written Agreement. Use of AI tools does not alter Service Provider's wonership of the deliverables, nor does it grant Client any additional rights beyond those expressly stated in this Agreement.

13. Governing Law These Terms shall be governed by and construed in accordance with the laws of Florida, United States, without regard to its conflict of laws principles.  

14. Changes to Terms We reserve the right to modify these Terms at any time. Any changes will be effective immediately upon posting on our website. Your continued use of our services after any changes indicates your acceptance of the new Terms.
